Said: The forms downloaded perfectly and were worth the momey, but I kept searching for the directions on how to use them and it turned out there were none. You must first call your local Court House and determine where to submit the notarized Petition for Adult Name change. You must wait until the Petition has been processed by the Court before you take the Application for new a Social Security Card to your local Social Security Office.
