David Money Said: The name says it all Easy Forms! I recommend this to anyone starting an organiztion. Easy Forms allowed me to prepare the paperwork myself rather than paying someone else to draw up the forms and prepare them. Over all it saved me time and money which is of great benefit for a start up company.

InA Hurry Said: Easy, fast download. Simple form, just what I neede. However, can't give it a "10" because the language was not gender-neutral and did not allow a way to quickly customize "he" to "she." Otherwise, it's a good value when you need a form in a hurry!
